Full Job Description
We are seeking a highly motivated, enthusiastic and experienced Flow Coordinator to join our MSE Bridging Service. The position will involve the assessment of patients who are fit for discharge, liaising with patients and families to ensure an understanding of the service as well as coordinating referrals and activities associated with the care of patients in their own home. The service is part of the MSE Integrated Care Department which is involved with several exciting projects and regularly generates opportunities for development
- To work as a part of the assessment team for Bridging Service, contributing to screening patients on the wards and developing care plans and protocols based on individual needs.
- To ensure patients identified for the Bridging Service understand the care they will receive and ensure that the care is coordinated and delivered to the best possible standard.
- The role includes implementing Activities of Daily Living (ADL) facilitation when and as necessary to identified patients referred to the Bridging service.
- Undertake specific duties involving direct and indirect care as identified by the multi professional team to support the delivery of effective healthcare to patients/ service users.
