The local teams work closely with local partner agencies such as substance misuse, mental health and liaison and diversion teams to offer a non-judgemental, patient focused approach which considers their immediate healthcare requirement, long term conditions and co-morbidities and the patients ongoing social and health needs.
  • PHLs values are embedded throughout the service, ensuring that we offer a caring and respectful service to all patients and partners but ensure we are accountable and work as part of the wider professional team that support these patients.
