Full Job Description

As a team our aim is to support people living with frailty and their relatives/carers when they are attending the acute hospital. The role of Frailty Clinical Pathway Coordinator is to ensure the smooth running of the frailty service, both on the Frailty Assessment Unit and the wider frailty caseload. To provide full and comprehensive admin support to the frailty MDT; this may include co-ordination of both clinical and non-clinical activities on the unit and within the Emergency department and wards. Duties may include, contact with front line service users, members of the public and other external stakeholders. This post will include organisation and maintenance of effective systems (paper based and electronic) and typing and preparation of correspondence. (these could be Medical or non-Medical staff)., Undertake clerical tasks supporting frailty MDT.Telephone enquiries directed to the frailty team.Take messages accurately.Assist in the collection of patient details and history.Uploading of clinical documentation.Sending of SBAR referrals to community services.Attending Teams meetings with Community services.Arrange and attend MDT meetings, minute these.Booking of Frailty Hot Clinic appointments.Holding investigation lists for Frail and HCOOP patients.Arrange follow up with patient and clinicians.Assist in the collection, collation and presentation of activity data required. Booking of teaching rooms,Booking of meeting roomsPreparing papers.Ensuring effective time management and forward planning the frailty service.
