Full Job Description

An exciting opportunity has arisen to join our Frailty Assessment team as band 4 Senior Therapy Support Worker. The role involves working alongside senior frailty therapists and the frailty multidisciplinary team to provide a rapid assessment of older adults, initiating services where required to safely avoid unnecessary hospitalisation and deconditioning.
The post holder would contribute to comprehensive geriatric assessment for individuals attending the Emergency Department, Frailty Assessment Unit and Frailty Same Day Emergency Care to achieve a positive outcome for the patients.
This post would suit an experienced therapy support worker or senior therapy support worker with an interest in working with frail older adults.
Additional working pattern details: Expected working pattern 8am-6pm 4 days per week - this can include weekends depending on shift pattern
The proposed interview date is 4th February, Contribute to service delivery by independently managing a caseload of delegated clinical and non-clinical tasks under the guidance of a qualified therapist.
To work as a proactive member of the Frailty team, contributing to comprehensive geriatric assessments by assessing, implementing and monitoring treatment plans of patients, performing specific clinical duties within levels of competency deemed appropriate for this post.
Demonstrate an excellent awareness of community pathways and referral processes to enable timely, safe discharges from the acute trust.
Support the learning and development of new starters and students.
Develop and maintain communication with a wide range of patients, carers, health care professionals and other departments as appropriate.
Perform administrative duties as required supporting the Frailty Therapy team and wider Frailty team.
Ensure equipment is available for treatment and responsible for checking it is safe for use prior to issue., Demonstrate knowledge of and undertake assessments of patients with non-complex needs e.g., Activities of daily living, mobility and transfers, using therapeutic techniques and principles of Occupational Therapy and Physiotherapy. Using these skills and knowledge develop a plan for discharge using clinical skills within scope of practice., Maintain accurate, comprehensive and timely clinical records of all care, advice and support delivered with clear reasoning for interventions in accordance with trust and national guidance.
- Plan and prioritise own workload in accordance with therapy standards and in conjunction with the responsible therapist.
- Monitor, assess and analyse progress and report back to qualified staff at regular intervals, participating in daily huddles.
- Demonstrate ability to work alone in domiciliary settings when required, assessing and managing risk - this includes driving trust vehicles where required.

-Demonstrate knowledge of and ability to effectively use, assess for and provide appropriate equipment.
-Demonstrate an excellent knowledge of manual handling techniques and risk assessment.
- Have knowledge and experience of general medical conditions and or clinical interventions pertinent to a caseload of frail older adults.
- Effectively communicate with patients, carers and relatives verbally (face to face or telephone), in written form and non-verbally.
-Ability to take into account and recognise situations that are distressing or emotional and respond accordingly.
- Communicate effectively and as an advocate where the patient's ability to communicate is compromised. Using effective interpersonal skills to communicate with patients and relatives to provide patient centred care.
- Maintain effective communication links with all levels of nursing and other health care professionals.
- Awareness of barriers to communication (e.g., perceptual, cognitive, physical) and employ strategies to overcome these.
- Ensure confidentiality and dignity is respected and maintained at all times.
