Ensuring viable GP services remain at the heart of local communities, providing sustainable and high-quality patient-focussed healthcare is our key purpose at Pennine GP Alliance. A fundamental part of this is delivering proactive health projects that trail new models of care delivery.

The Physical Activity Champion will play a pivotal role in promoting and encouraging physical activity within Calderdale. They will work collaboratively with General Practice staff and patients across all five Primary Care Networks, along with Active Calderdale to create a supportive environment that fosters healthy lifestyles., A primary role of the Physical Activity Champion will be to support GP practices in their journey to becoming Active Practices, along with identifying opportunities and strategies to increase the number of physical activity conversations taking place within General Practice.

This role would suit someone with experience of working with healthcare professionals and or previous experience in the NHS, social care or relevant field (including unpaid work).

Pennine GP Alliance is a member-led organisation made up of 19 GP practices in Calderdale who form 5 Primary Care Networks (PCNs), collectively serving over 223,000 registered patients.

We work extremely closely with general practice but also in partnership with other local healthcare providers with a purpose to ensure viable GP services remain at the heart of local communities, providing sustainable and high-quality patient-focused healthcare.
