ThePhysical Activity Champion will play a pivotal role in promoting andencouraging physical activity within Calderdale. They will work collaborativelywith General Practice staff and patients across all five Primary Care Networks,along with Active Calderdale to create a supportive environment that fostershealthy lifestyles. This role would suit someone withexperience of working with healthcare professionals and or previous experiencein the NHS, social care or relevant field (including unpaid work). Main duties of the job A primary role of the Physical Activity Champion will be to support GP practices in their journey to becoming Active Practices, along with identifying opportunities and strategies to increase the number ofphysical activity conversations taking place within General Practice., Enhance understanding of what constitutesphysical activity with General Practice staff. Work with Active Calderdale to gain localintelligence and insight across the team to inform decision making anddetermine priority areas to target. To build relationships with, and ensurephysical activity is integrated into local structures. Promote the role of physical activity andtraining for relevant workforces. Recognise the complexity of enabling peopleto get physically active and that the wider determinants of health affectpeople differently. Enhance local capacity andleadership in physical activity by supporting the development, delivery, andevaluation of training and capacity-building programs tailored to our patientpopulations. Collect and analyse data on patient physicalactivity levels and program outcomes.

Essential

  • Educated to undergraduate level or equivalent.
  • Desirable
  • Qualifications in physical activity and / or health promotion.
  • Experience Essential
  • An understanding of other avenues of support open to patients and an ability to signpost patients to other professionals ensuring a continuity of care.
  • Experience in health behaviour change or coaching.
  • Experience of partnership / collaborative working and of building relationships across a variety of organisations.
  • An understanding of the complexities around barriers to physical activity.
  • Knowledge around the role of physical activity as a tool for prevention and management of long-term medical conditions.
  • Experience of working with or in general practice.
  • Confident using IT systems, in particular Teams and the Microsoft package.
  • The ability to research and work within best practice.
  • An understanding of health inequalities and measures that can be put in place to combat them.
  • An effective organiser, influencer and networker who is willing to build relationships and engage stakeholders.
  • Continued commitment to improve skills and ability in new areas of work.
  • Desirable
  • Experience in the use of clinical systems such as EMIS or SystmOne.
  • An understanding of, or experience of working within the varying localities in Calderdale.
  • Other requirements Essential
  • Disclosure Barring Service (DBS) check.
  • Maintain confidentiality at all times.
  • Full UK driving license and access to a car.
  • Desirable
  • Community lone working experience.
  • Flexibility to work outside core office hours.
