We are excited to offer an innovative opportunity to enhance primary care delivery to care homes in the borough of Ealing. As a Care Home GP, you will value having dedicated time in your allocated care homes to build relationships with residents and staff and provide consistent, high-quality care. This is far more enjoyable than the pressures of ad-hoc care home visits which you may have experienced in other roles! Trust us on this, we have experienced that too. As a valued member of our multidisciplinary team, you will receive robust support from fellow GPs and an experienced group of nurses, including advanced nurse practitioners, prescribing pharmacists, and administrators. Weekend working opportunities can be discussed if you are interested.
  • Patient Population: We provide enhanced primary care to over 1100 patients living in nursing and residential homes in Ealing.
  • Rewarding Work:Engage in meaningful work with opportunities to develop your expertise in frailty, dementia, multi-morbidity, and polypharmacy through direct patient care and via our weekly MDT meetings attended by secondary care and community colleagues such as Marie Curie nurses.
  • Patient-Centred Care: Work with care home residents and their dedicated care home staff who will support with care management plans and ongoing monitoring.

    Experienced GMC registered GP(s).
  • Care home or Medicine of the Elderly experience preferred
