Full Job Description

An opportunity has arisen for a motivated, patient centred individual to join the Stroke Specific Early Supported Discharge Team, to support patients in the community who have recently suffered a stroke in the East BCUHB area.

This post is to support Physiotherapy and Occupational Therapy as part of a wider supportive multidisciplinary team including speech and language therapists, lead by our Stroke Specific Consultant therapist. Our team strives to provide efficient and holistic assessment and therapy intervention to patients living across Wrexham and Flintshire for up to six weeks.

The position will assist our team in providing specialist stroke rehabilitation tailored to each patient's needs, working towards the goals that are important to them, to optimise their quality of life after leaving hospital.

The post holder will be responsible for carrying out therapeutic interventions delegated by the qualified therapists, in the patient's place of residence.

This may range from helping a patient prepare a simple meal to helping a patient return to their usual activities such walking to the shops. Patients may need support with their cognitive function such as their orientation and task sequencing.

You will be given the opportunity to learn and develop with support from specialised and experienced colleagues.,

  • Gathering of baseline information to assist the Therapy Assessments.

  • Carry out therapeutic interventions following initial assessments carried out by the Physiotherapist and Occupational Therapist.

  • Interventions will include progression of transfers and mobility, delivery of exercise programmes including exercise classes and practicing activities of daily living such as washing and dressing and meal preparation.

  • As a Band 4 TI you will also work independently to promote wellbeing, safety and independence within essential activities of daily living, by providing instruction, advice and treatment to the service-user and/or their carers.

    The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
