Full Job Description

The Stroke Early Supported Discharge Team provides stroke specific assessment and associated care to patients discharged from hospital. The Generic Rehabilitation Technician supports the therapists to provide specialist assessment and treatment. The service is currently a 5 day service with plans to provide a 7 day service in the future, with staff required to work on a rotational basis. There may be a need to provide countywide cover on occasion.

Visiting patients in their home to provide rehabilitation and therapy as directed by the registered occupational therapist, physiotherapist and speech and language therapist within the team.

To support stroke patients to become more able, independent, and support in the self management.

Main duties of the job

The stroke team is a multi disciplinary team that works across the county to provide rehabilitation, support and education to people following stroke.

The team works collaboratively with other teams across the stroke pathway in Cornwall. The main aims of the service are:

To encourage patients and their carers in an active approach to regaining and maintaining personal independence. This requires empathy, sensitivity, and good interpersonal skills and may require motivational skills with some patients.

To communicate effectively with patients and carers and provide patient centred care, to encourage self-management, maximise rehabilitation potential, and to ensure understanding of the condition.

To liaise with the therapists as necessary regarding patient care. In particular highlighting any variations to expected outcomes from treatment programmes you deliver.

To accept patients referred by an Occupational Therapist/Physiotherapist/Speech and Language Therapist and carry as own patient caseload. Treat patients according to set treatment protocols, monitoring patient progression, make alterations (within protocols and scope of practice) to patient treatment programmes as required.

To fully participate in the inter-professional evaluation of the rehabilitation programme, to ensure patients meet their on-going health & wellbeing needs.
