Full Job Description

Leading on existing recall systems and introducing new Long Term Condition recall systems to providean effective and efficient recall system to patients including but not limited to:o Chronic disease management such as diabetes, asthma, heart disease etc.o Screening programmes - support the follow up of patients called through national programmes e.g., Cancer screening: Cervical, Breast, and Bowel.o Vaccination - support the practice with various vaccination programmes.o Health Promotion including NHS Health Checks and Diabetes Prevention Programme.o Monitor patient registers to ensure patients are called in, in a timely manner.o Taking a lead role to monitor performance against Practice standards or frameworks such as the Quality and Outcomes Framework (QOF). Working with clinicians and the management team to develop clinical system searches and reports and other information systems as necessary.o To monitor and amend where required patient records to ensure records are kept up to
date, this will include updating personal details, coding to update the medical care record, asking patients for information opportunistically when they are on the phone and cover reception at the practices if addition support is needed
