Full Job Description

This is an exciting new post of General Practitioner with a special interest in Neurodevelopmental disorders for the Neurodevelopmental pathway where 10 sessions are available. The post was developed to help support this pathway, secondary to an increase in referrals of patients with neurodevelopmental conditions primarily ADHD and ASD. These multidisciplinary pathways for the assessment, diagnosis and management of children and young people with ADHD and ASD, are supported in conjunction with nursing and psychology colleagues. The pathway involves obtaining written information / screening questionnaires from parents and school. A decision is then made with regards to the need to carry out further direct assessment by a neurodevelopmental team practitioner. After this point the Doctor will contribute to the diagnostic assessment & treatment / management. The Doctor will also support management of associated difficulties of patients, for example sleep difficulties, anxiety, school
difficulties and challenging behaviours etc.
The successful candidate will join an experienced community multidisciplinary Specialist CAMHS team. They will join two CAMHS Consultant colleagues, a Neurodevelopmental Doctor and a specialty doctor working from Bury community CAMHS based at Fairfield General Hospital., The initial psychiatry appointment will be to make the formal diagnosis of ADHD after the previous assessment in which an ADHD diagnosis was felt to be indicative. Management options will be discussed and could include signposting to a post diagnostic service, discussing and commencing ADHD medication and/or managing co-morbidities. Medication would be monitored in follow up clinic with physical health/ side effect monitoring. After stabilisation, patients would be followed up in the nurse-lead ADHD clinic. Consultation for the ADHD clinic nurses would be required on a regular basis.
The successful candidate will join other team members in contributing to the ASD care pathway. The candidate will be supported in developing expertise in assessing and diagnosing children and young people with ASD.
+ The post holders will not have any out of hour's responsibilities.
+ The successful candidate will be expected to accumulate their own out-patient load from new referrals and help out with known patients in out-patients, and pick up ongoing patient management through the team as appropriate.
+ The successful candidate is expected to contribute to the bio psychosocial model of care with the other members of the multi-disciplinary team. This includes history taking, mental state examination of all patients and the physical examination of outpatients where appropriate. The treatments may be physical, pharmacological, psychological and social as appropriate., · Referrals for a neurodevelopmental assessment are submitted by the child / young person's GP or school to Bury CAMHS. The pre-referral paperwork (SNAP questionnaires /parent + teacher checklists and developmental history) is triaged by the neurodevelopmental team to determine if an assessment is indicated. If an assessment is indicated the young person is seen for an initial assessment. If assessing for ADHD, a QB test is arranged and if assessing for ASD, an ADOS assessment is arranged. All the information gathered and the results of these assessments are then discussed at either the ADHD MDT meeting or ASD MDT meeting, where an outcome is agreed. A feedback appointment is then arranged with the family and young person. If the child / young person meets the diagnostic criteria for ADHD they are added to the Psychiatry waiting list for further input from a Doctor. Patients who are diagnosed with ASD are discharged back to the care of
their GP unless there are significant additional mental health needs requiring further input from CAMHS. If further input is required, then the clinician with the appropriate skills will provide input to the patient. For example input from a Psychiatrist if the needs are Psychiatric or medication needs to be prescribed, or input from a Clinical Psychologist if psychological therapy such as CBT needs to be provided etc.
+ Contribution to the ADHD care pathway - The initial psychiatry appointment will be to make the formal diagnosis of ADHD after the previous assessment / meetings in which an ADHD diagnosis was felt to be indicative. Management options will be discussed in accordance with NICE guidelines. These could include signposting to First Point (post diagnostic service), discussing and commencing ADHD medication and/or managing co-morbidities. Medication would be monitored in follow up clinic with physical health / side effect monitoring. After titration and stabilisation, patients would be followed up in the ADHD monitoring clinic. Consultation and supervision for the ADHD clinic nurses and participation in duty prescriber rota will be part of the role.
+ The post holder will not have any out of hour's responsibilities but will be expected to contribute to the day time duty medical rota in Bury CAMHS. This will entail assessment of / intervention for urgent CAMHS patients presenting to the service requiring Psychiatric input. The Duty Consultant Child & Adolescent Psychiatrist will provide supervision for any such input.
+ The post holder will be expected to accumulate their own out-patient load from new referrals and help out with known patients in out-patients, and pick up ongoing patient management through the team as appropriate.
+ The post holder is expected to contribute to the bio-psychosocial model of care with the other members of the multi-disciplinary team. This includes history taking, mental state examination of all patients and the physical examination of outpatients where appropriate. The treatments may be physical, pharmacological, psychological and social as appropriate. The CAMHS team offers various psychological therapies such as CBT, family therapy, IPT and DBT.
+ The post holder will be expected to adhere to the policies and procedures in support of the caseload, maintain appropriate professional records, confidentiality, administration of out-patient clinics and the appropriate admin/discharge and referral procedures.
+ There is an on site Duty Consultant every day of the week to support and provide consultation to the Specialty Doctor with respect to any urgent assessments / matters. This is separate to the weekly clinical supervision sessions which will be provided by the supervising Consultant.
· The post holder will be expected to utilise the Care Programme Approach, and also the New Mental Health Act, the Children Act and the Mental Capacity Act when required.
· The post holder will work in partnership with other professionals/agencies involved in the care of young people, across departmental and organisational boundaries. This includes providing regular consultation to other agencies involved in the care of young people.
· The post holder will attend regular (weekly) supervision meetings with the Bury CAMHS Lead Consultant, Dr. James Glass. This provides a forum for discussing and receiving advice with respect to their clinical cases, learning from their Consultant supervisor and caseload management.
· The post holder will provide mutual prospective leave cover for annual and study leave with the other Bury CAMHS specialty doctor.
