Full Job Description

You will support and advocate on their behalf, ensuring as far as possible their health and social needs are met. You will work closely with other agencies and be proactive in establishing links with new partners.
You will play a key role in supporting the most vulnerable in our community to reduce barriers and enable them to engage in Forward Leeds care. As part of the Harm Reduction Social Support Team, you will work out in the community to provide assertive outreach to drug and alcohol users across Leeds and work with all partners attached to Forward Leeds and external services.
You will be part of a dynamic and responsive team so a positive attitude and a willingness to work flexibly to meet the needs of our service users is essential. You will be working in accordance with the values of St Anne's Community Services and the Forward Leeds consortium., Working in partnership with Forward Leeds colleagues, and other community organisations, to identify people not currently under Forward Leeds care that would benefit from support around their drug and alcohol use.
Assertively outreach identified individuals to reduce barriers to accessing support around their drug and alcohol use.
Complete comprehensive assessments with people who use substances. These will be conducted in their homes or an agreed community venue. You will then arrange continuity of care for the person to seamlessly access Forward Leeds support and support them along this journey.
