Full Job Description

An exciting opportunity has arisen for an enthusiastic and self motivated HCSW to work within the Discharge Lounge based at Nevill Hall Hospital. Applicants need to be professional, caring, friendly, compassionate, and have a degree of flexibility to meet the needs of the service.

The role involves transferring patients from base wards to the discharge lounge, provide care whilst they are in the lounge and ensure the patients are discharged safely., This team ensures patients discharges are expedited by transferring from their ward to a more appropriate area. The service identifies appropriate patients, liaises with their wards and transfers the patients to the discharge lounge, ensuring all belongings are with the patient. This is an opportunity to work with a small and dynamic team supporting the overall flow of the organisation.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
