Full Job Description

To support patients who have suffered major traumatic brain injury throughout their hospital stay and post discharge, with a particular focus on the A&E Department and the Critical Care Departments . To navigate patients and families on to further support services as required and in conjunction with the multidisciplinary team.

To develop strong cohesive cross collaborative working relationships with medical staff, ward managers, nurses, therapists and those involved in direct support of the patients and family members/carers.
The post holder will take a proactive approach to arranging and supporting patients and their carers through their care pathway, including goal planning and discharge planning meetings, ensuring patients and carers are equipped with knowledge to manage condition and are aware of escalation pathways if needed.
The post holder will be a source of professional advice for district nurses, care agencies and other staff within the region, and will act as a patient advocate at all times.

We also offer a range of additional in-house and externally provided courses and bitesize programmes through our Leadership and Improvement Team., It is now the policy of South Tees Hospitals NHS Foundation Trust that all successful candidates who are new to the Trust who require DBS clearance for the post they have been offered, are required to pay the cost of their DBS. The method of payment for this is via salary deduction from your first month's pay.

We are looking for an enthusiastic and highly motivated nurse,
You will have a proven ability to lead, inspire and engage all members of the multi-disciplinary team.
You will be a team player, a good communicator, have excellent organisational skills, together with significant proven in depth clinical knowledge and experience of caring for patients who have sustained a head injury and various neurological conditions.
You will work closely with all members of the multidisciplinary team within the Neurorehabilitation service. you will be working across various departments to support patients and relatives through their journey following head injury.
Network with Accident and Emergency Departments, Intensive Care and High Dependency Units, Neurosurgical and rehabilitation wards to identify and assist those requiring support.
You will need to have problem solving skills and be able to enhance patient pathways with innovation and improvements based on current research and guidance.
