Full Job Description

The postholder is responsible for the leadership, management, and development of all Hospice Community Services, under the supervision of the Director of Clinical Services. This will be in line with the new overarching Hospice Strategy. This will align to the needs of patients, particularly in the community setting, considering the local, regional, and national policy and guidance direction and how we as a service meet those needs.

The post holder will provide management and leadership support to the team of community Clinical Nurse Specialists (CNS), community Fast Track Home Care domiciliary service, Wellbeing Services and to include any new community outreach services as developed.

Whilst the above list specifies the direct service responsibilities of this role, it is imperative to recognise the significance of key relationships across the Hospice as well as the collective shared responsibility to meet the needs across all clinical services. The post holder will provide specialist palliative care advice to internal and external professional colleagues, stakeholders, as well as patients, their families and carers., For a full and comprehensive job description and person specification, please read through the attached documentation.
