Come and join the Hounslow Place based team. We are a team that is excited by the opportunities to support people to live independently with a focus on integration and reducing inequalities., NHS NW London Hounslow Place Team is looking for a dynamic and enthusiastic colleague to take the lead on developing integrated neighbourhood teams across the Borough. They will also be the local lead on primary care and will work in close collaboration with the full range of statutory and non-statutory partners. The ideal candidate will have current and demonstrable knowledge of the national agenda for integrated neighbourhood teams and primary care and be able to demonstrate how they will use their understanding to make an impact in Hounslow. A thorough understanding of population health management approaches will be needed to help drive the improvements needed across the Borough to reduce inequalities and deliver the ICB priorities. Experience of delivering complex transformational projects, including working with a diverse range of stakeholders to achieve the desired outcome will be essential along with good project management and problem solving skills.

NHS North West London Integrated Care Board (NHS NWL ICB) is a statutory body responsible for planning and allocating health and care resources to improve the lives of people in eight boroughs across North West London: Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, Royal Borough of Kensington and Chelsea and Westminster.

The local population is large and diverse, with a wide range of health and care needs that are evolving all the time. Working with our partners including Councils, NHS provider Trusts, GPs and voluntary and community organisations we plan and allocate NHS resource to deliver a wide range of services including urgent and emergency care, mental health, primary care, elective hospital services and community care.

All ICB roles work collectively to ensure our residents and patients have access to the right services when they need them as well as focusing on prevention and health promotion, supporting residents to take greater care of their own health and live longer, healthier lives.
