Full Job Description

This exciting opportunity has arisen within Community Health Services (CHS). The Head of Service for Advanced Clinical Practice will work alongside the CHS Associate Medical Director, Executive Director and Head of Nursing, with the support of the Directorate Leadership Team. The post holder will be accountable for the strategic, operational and clinical leadership of the CHS Advanced Clinical Practitioners. The post holder will be responsible for ensuring effective governance of the diverse range of clinical services provided in CHS with particular focus on the Medical and ACP provision for the Directorate. The community hospitals are adult community bed-based services with 250 inpatient beds across 13 wards providing sub-acute step down and rehabilitation services across 9 wards, 2 stroke rehabilitation wards and 2 wards delivering discharge to assess pathway beds provided from seven community hospitals across LLR. The post holder will also provide clinical and professional leadership to the CHS Virtual Ward services working alongside the Operational Leads., Key areas of delivery include:

  • To provide professional and clinical leadership and direction across the Directorate including the inpatient community wards and virtual Wards e.g. Frailty.
  • To be responsible for developing, implementing, and reviewing the Advanced Clinical Practitioner / Medical service ensuring that the service is delivered equitably and to a consistently high standard across Community Health Services.
  • Provide a leading role in appropriate aspects of the ACP service across the directorate working in partnership with the Consultant Geriatricians to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care in community hospital in-patient settings and the Frailty Virtual Ward.
  • Line Management of the two Lead Advanced Clinical Practitioners for Community Hospitals Provide clinical leadership to the Advanced Clinical Practitioners for the Virtual Ward offer.
  • Leadership, planning and delivery of staffing including recruitment, retention and deployment.
  • Be highly motivated and responsible for ensuring robust local accountability for performance, quality, safety and finance across the team.
  • Will have a clear workplan of 20% clinical and 80% leadership/education/audit and research to reflect the 4 pillars of advanced clinical practice.
  • Participate in the directorate silver oncall rota., patterns in line with our flexible working policy.
