Health Assessment Nurse

Responsible for identifying and assessing the health needs of Asylum Seeker and Refugee adults and children, and developing, implementing and evaluating programmes of care with onward referral to other health providers, disciplines and agencies, as necessary., Provide a holistic assessment of health care needs for Asylum Seekers and Refugees attending The Haven clinic, and promote the uptake of health screening and preventative health care.
Assess needs across a spectrum of age groups, some with diverse and complex health needs, with referral onto appropriate Specialist Services as required.
