Job Summary
To assist the registered nurses in meeting patients' individual needs in support of their care plan.
Maintains and promotes patient independence and work in a person-centred manner
Respects the dignity, privacy, and wishes of the patients.
The post holder will be accountable for their behaviour whilst on duty and will be responsible for reporting anything untoward, which could interfere with the quality of care provided, to the Registered Nurse in charge.
The post holder will be encouraged and supported in drawing up a personal development plan and the meeting of training needs.
Principal Responsibilities
1. To assist patients with all personal care needs including, undressing, washing, bathing and toileting needs.
2. To assist patients with mobility problems and other physical disabilities, including incontinence and help in use and care of aids and personal equipment.
3. To support and care patients who are at the end of life.
4. To engage in the promotion of mental and physical well-being of patients through talking to them, taking them out, sharing with them in leisure and purposeful activities.
5. To strip the beds and all domestic tasks as required.
6. Where required, to assist with patient's laundry.
7. To read and draft reports and take part in staff and patient meetings and in training activities as directed. Failure to attend training may mean we are unable to provide shifts until the training is up to date
8. To perform such other duties as may reasonably be required.
9. To be conversant with and work primarily to meet the needs of the patients in a timely and courteous manner
10. To always comply with the organisation's policies and procedures.
11. To report to the nurse- in-charge any significant changes in the health or circumstances of a patient.
12. To encourage patients to remain as independent as possible.
13. All staff are required to undertake the All-Wales Induction Framework (unless they have already done so of hold and equivalent level of qualification as recognised by Social Care Wales) and register with Social Care Wales.
14. Under the direction of the Registered Nurses support and assist patients in identifying risks and recording the support plan which minimizes and manages the identified risk
15. To agree and record with the patient their needs wishes and preferences and how these are met.
16. Record appropriately all decisions which relate to patients who lack capacity.
17. Involve and consult with residents, their families, representatives, or advocates to ensure that the resident's views are expressed and acted upon.
18. Attend reviews of care as appropriate.
19. Update and continually improve practice by a mutually agreed method which may include attendance at staff meetings, learning support briefings and one-to-one supervision.
20. Take responsibility in conjunction with line manager for identifying further learning.
21. Be aware of requirements regarding codes of practice and relevant codes of conduct where appropriate issued by professional bodies.
22. Promote good practice and be vigilant in recognising and reporting practice which does not meet the defined standard.
23. To assist and contribute to any regulatory body inspection or monitoring visit.
24. To act always, to safeguard residents from any form of abuse and to report such concerns immediately as required in the Wales Safeguarding Procedures.
25. Undertake any other reasonable duties deemed necessary by the Responsible Individual
