Full Job Description

We are looking for individual who would like to be part of a dynamic and forward thinking team. Do you enjoy the challenge of a wide variety of work? Could this be you?

The role of a community HCSW is to visit patients at home to provide nursing interventions under the direction of registered nursing colleagues. The focus of the role is to support admission avoidance, to promote independence and to assist in the effective management of patients with long term conditions., To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients' privacy and dignity at all times.

To act as key worker for patients under the direction of the registered practitioners.

To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.

To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.

To be responsible for progressing patient's treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient's progress within the team

To perform a range of clinical nursing skills as directed by the registered practitioners:

Skin care/Pressure Ulcer prevention

The posts would suit someone interested in integrated working and wanting to achieve the best for their patients. They will work within a multi -professional team to provide individualised patient care.

We are looking for a motivated individuals, who are dynamic and keen to work as part of a team. The successful candidates will need to have some relevant experience, good communication skills and the ability to work well as a team member.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
