Full Job Description

We are looking to recruit enthusiastic Health Care Assistant who have a genuine interested in providing safe and effective care for patients at home. The successful candidate must have demonstratable experience, a wide range of clinical skills and will be confident to work without direct supervision. Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change. We invite Heath Care Assistants with previous community experience to apply, but this is not essential as successful candidates will be offered training and support., To assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting., To work as part of an integrated Community Nursing Service in providing specific care to a patient in the community setting as written in the care plan.

To provide personal care and support to palliative patients in their own homes.

To work alone and unsupervised and the ability to work autonomously.

To be responsible for the ordering of clinical supplies and stationery for the Community Nursing Team.

To work in accordance with Trust Policies and Procedures.

To maintain high standards of personal hygiene adhering to Health and Safety Guidelines and Infection Control Guidelines within the trust.

To respect confidential information during the course of your work and refrain from discussing such information without the consent of the patient/client or a person entitled to act on his/her behalf, except where disclosure is required by law or by the Order of a Court or is necessary in the public interest.

Flexible working will be considered for all roles.
