Health Care Assistant

If you have NVQ Level 2, experience ofworking in a health or social care environment and excellent communicationskills this may be the position for you. You will work autonomously,predominantly in the patients home, but you will also be an integral member ofthe wider, multi-disciplinary team. The post is for hours per week that maybe worked flexible over a rota period. Main duties of the job Thepost holder will provide direct personal nursing care, primarily in the patientshome and on occasions in other settings such as Community Hospitals,Residential and Nursing homes and Eden Valley Hospice. Thepost holder will provide practical nursing care, emotional, social andpsychological support to individuals with palliative care needs, their familiesand carers. • Knowledge of palliative care issues
• An understanding of multi-disciplinary working and working with other organisations District Nurse Teams.
• Enhanced communication skills to work with patients, families and health / social care professionals.
• Ability to work in a complex environment, often in isolation and demonstrate effective coping mechanisms.
• Quality focused with an innovative approach to solving complex problems.
• Diplomacy
• Sensitive to the needs of those affected by life-limiting illness.
• Flexibility to work various shifts patterns over a 24 hour period 7 days a week.
• Ability to respond to changing work shifts.
• Must be able to travel autonomously across the geographical area covered by the service.
Desirable IT Skills-knowledge of patient information systems EMIS Qualifications Essential
