Welwyn Hatfield Integrated Community Team are seeking a motivated and enthusiastic Health Care Assistant to join our friendly, supportive and award winning Integrated Care team. You will be working alongside our trained nursing and therapy staff assisting them in the day to day care and management of our patients to ensure they receive safe, high quality nursing care. As a Health Care Assistant, you will work alongside our diverse team comprised of nurses, paramedics, physiotherapists, occupational therapists and technical instructors. We work to provide evidence based nursing care to patients in their own homes. Our service operates 8am - 10pm 365 days a year.   • To work within a team that provides an assessment and care service to patients with nursing / health needs.
  • To visit patients in their own homes and provide appropriate care and support as defined in the Care Plan. This will include aspects of personal care, hygiene, uncomplicated nursing procedures, exercise, activity and treatment programmes prescribed by therapists. The aim is to rehabilitate and maximise the patient's potential and where appropriate to support the carer.
  • To support the Hospital at Home team with blood sampling
  • To support and enable patients in a range of activities to promote independent living and monitor their well-being.
  • To undertake administration and clerical tasks that are associated with the care process including record keeping and use of computer. Word versions of documents are available if needed to allow for any adaptations to be made i.e. font, colour, background, convert to Read Aloud.
