Health Care Assistant (Overnight) - Palliative Response Team

The purpose of this role is to deliver care and support to patients, families and carers at home for those identified as requiring care and support at end of life.,

• Competently working alone, or with a registered health professional (nurse or paramedic), as required, with patients and their families in the home environment with support and guidance from qualified professionals.
• Respond to all calls and requests for patient visits, speaking with patients and relatives as required.
• To adhere to the lone worker policy and procedure.
• Working collaboratively with hospice and community partners.
• Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.

We will support people of all ages who are affected by a progressive life-limiting illness (and those who care for them), to live as well as possible by providing choice and ease of access to a wide range of palliative care and bereavement services across Buckinghamshire and Hertfordshire. We provide care and support for adults and children diagnosed with cancer and other life-limiting illness, and their families, visiting day and night.
