Full Job Description

The role of Rapid Personalised Care Service Healthcare Assistant ( RPCS ) is in place to provide end of life care to patients in their own homes in the final weeks of life. You will work in the community so a car and driving licence will be essential. KEY ACTIVITIES

  • Competently working alone, as required, with patients and families in the home environment with remote support and guidance from the RPCS Management in adherence to lone worker policy and procedure.
  • Working collaboratively with hospice and community partners.
  • Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.

We will support people of all ages who are affected by a progressive life- limiting illness and those who care for them, to live as well as possible by providing choice and ease access to a wide range of palliative care and bereavement services across West Hertfordshire and Buckinghamshire. We provide care and support for adults and children diagnosed with cancer and other life-limiting illness, and their families, visiting day and night.
