Our practice: 9,200 patients 6 GP Partners and 2 salaried doctors Partnership owned premises Wide range of clinical services including minor surgery, coils and implants, care homes DES, anticoagulation, insulin initiation Clinical pharmacists undertaking polypharmacy reviews, prescribing support and a chronic pain clinic GP training practice Medical student teaching Excellent support from experienced clinical and admin teams We are a leading member of the Cardiff South West Cluster working with local partners to explore how best to work in a collaborative way to maximise our patients wellbeing and
benefit from the following: MDT working with partner organisations and discharge Hub Community connectors and social prescribing service Paediatric teaching clinics and MDT discussions Community garden Specialist IT support Our doctors are supported to maintain a range of external interests including Clinical Director roles in Diabetes, Cancer and Palliative Care. The GP Practice is often first point of contact for healthcare. The aim is to provide an easily accessible route to care, whatever the patients problem. Job Summary To assist the GPs and practice nurse team in the service and delivery of care management of the practice population. The job holder will work under indirect supervision of the clinical team. Key responsibilities Clinical practice To deliver the highest standards of care to patients Carry out baseline observations such as blood pressure, temperature and pulse rate and
record the findings accurately Support the wider clinical team with chronic disease management Carry out BMI checks Act as a chaperone as required Undertake venepuncture Administer vaccines flu, pneumococcal, shingles, B12 etc. Carry out ECGs Provide support during minor surgery as required Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Communication Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Anticipate barriers to communication and take action to improve communication Maintain effective communication within the practice environment and with external stakeholders Act as an advocate for patients and colleagues Team
working Understand own role and scope and identify how this may develop over time Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence Prioritise own workload and ensure effective time-management strategies are embedded within the culture of the team Discuss, highlight and work with the team to create opportunities to improve patient care Administrative Duties Participate in the stock control and ordering system Ensure clinical rooms are kept stocked
