Full Job Description

The post-holder will work within a community nursing team visiting patients in their home including care settings to deliver delegated nursing care. They will assist the registered staff in providing a highly specialized Community Nursing Service to adults. This involves assisting the registered staff in ensuring appropriate pathways of care for patients who require those services. To contribute to the assessment and the treatment of individual patients on the caseload and communicate any changes in care needs to the registered staff. To communicate with patients, relatives, carers and health care professionals to assist the registered staff in ensuring an integrated, holistic approach to patient care.

  • Able to work alongside a multidisciplinary group of health professionals.
  • Able to work independently and autonomously undertake clinical and non-clinical tasks.
  • Possessing observational and communication skills which complement your role.
  • Possess basic computer skills.
  • Willing to develop in your role and adjust your responsibilities as the service grows and transforms.
  • Able to work flexibly on a roster basis.
  • A car driver with access to a car.
