Merthyr Tydfil Older Adult Community Mental Health Team have an exciting opportunity for a Band 3 Health Care Support worker to join their team. We are looking for a dynamic individual who has a passion for working with older adults with mental health support needs. You will be responsible for supporting the individual with providing 1:1 care under the supervision of a qualified care coordinator; monitoring mental health wellbeing and reporting back to the care coordinator, as well as supporting the individual with group sessions or individual therapy. The HCSW will work alongside unpaid carers and 3rd sector agencies/care providers., To support the provision of Health and Social Care to the people of RCT and Merthyr who are experiencing a mental health issue within the context of a multi-disciplinary, multi agency team and support the delivery of evidence based mental health care to clients in a community setting.The post holder will deliver specific components of the patients care plan as directed by the registered nurse and Care Coordinator.The post holder will be required to work without direct supervision for all or most of their shift.The post holder will be expected to assist with the organization and facilitation of monitoring physical health needs and running therapeutic groups.
