Full Job Description
This is an exciting opportunity for a permanent B3 Health Care Support Worker to join the award winning Health Liaison team and work with adults with Learning Disabilities to support their access to primary and secondary care.,
- To provide day to day care for service users under supervision of a Clinician which adheres to best practice, standards and guidelines, and contributes towards the service users care and treatment.
- Work in partnership with service users, other health and social care colleagues and services, to achieve the optimum health outcome for clients.
- To be part of the MDT in providing a high and consistent level of holistic care, maintaining the dignity and equality of the individual.
The successful candidate will have experience of working with people with Learning Disabilities, be a team player and be motivated and committed to the improvement of the service and working alongside other multi-disciplinary team members., The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
