Full Job Description

An exciting opportunity has arisen within the Knowsley Community Learning Disability Team for a Community Learning Disability Health Care Support Worker to support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities.

The post holder will assist in the delivery of care plans and programmes by - implementing programmes and reporting outcomes, contributing to the recording of plans/programmes and providing therapeutic activity where agreed and appropriate in peoples homes or in the community.

Acting as a nominated worker for individual clients who have learning disabilities and may also display behaviours of concern including physical and verbal aggression, changes in mental health, complex physical health needs. Facilitate and support the development of appropriate personal care and independence with activities of daily living. Support self-help, communication and social skills acquisition

The service model is one of supporting people, whose behaviours challenge services, working closely with specialist practitioners and other health professionals to support this complex group of individuals to remain in their community setting and, where an appropriate admission has occurred, to be discharged promptly and safely in line with national Transforming Care strategy.

Travel across the Knowsley area throughout the day is required.

To promote and champion the rights of people with learning disabilities, working with the current service philosophy and values as detailed in Transforming Care for people with a learning disability

Support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities.

To assist the Enhanced intensive support part of the LD community team with care plans and programmes by :

Implementing programmes and reporting outcomes.

Contributing to the recording of plans and programmes.

Act as a nominated worker for individual clients who have learning disabilities and may also display behaviours of concern including physical and verbal aggression, changes in mental health, complex physical health needs

Facilitate & support the development of appropriate personal care

Facilitate & support self help, communication and social skills acquisition

Assist and promote independence with activities of daily living

Facilitate recovery by assisting participation in social, vocational, recreational and leisure activities

Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff

Demonstrate a range of communication skills & techniques to enable positive communication to take place with individuals who may have complex communication needs.

Under the direction of the qualified practitioner, participate in the delivery of planned care within a community environment. The post holder will undertake a range of duties which include:-

  • Facilitate and support the development of appropriate personal care

  • Facilitate and support self help, communication and social skills acquisition

  • Assist and promote independence with activities of daily living

  • Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behavioursunder the supervision of qualified staff

  • When required support the service user to access other health services and ensure reasonable adjustments are implemented

  • Facilitate group work as directed by the qualified Nurse Practitioners

  • Monitor mental and physical health and report outcomes to the qualified practitioner

  • Participate in regular supervision, engage in reflective practice and be committed to continued development

  • Provide information to enable the effectiveness of care to be evaluated.

  • Ensure appropriate records are maintained under the direction of the qualified practitioner.

  • Contribute to the involvement of service users, and carers/relatives in the delivery of care

  • Provide reassurance and support to service users and carers/family

  • Work collaboratively within the Care team to ensure that service users needs are met

  • Recognise and respond appropriately to urgent and emergency situations when directed by qualified practitioner

  • Contribute to the development of services

  • Contribute to the effective and efficient use of resources

  • Understand and adhere to Trust policies, procedures and clinical guidelines

  • Report any concern regarding service user care to the Qualified Practitioner

  • Contribute to service improvement initiatives to enhance quality of service user care

  • Participate in service user and public involvement activities

  • Support individuals equality, diversity and rights
