Full Job Description

An exciting opportunity has arisen for a caring and enthusiastic Band 3 Health Care Support Worker to join the Hospice at Home Team Evening Service, under the supervision of the Hospice at Home Team Leader. This is a 7 day service and the post offers a 16 hour, part time, permanent contract, primarily covering 6pm-10pm, but please note staff may be asked to cover other shifts on occasions to meet the needs of the service., We are delighted to offer this exciting opportunity, for experienced Health Care Support Workers to join our Hospice at Home Service which supports patients living in Wrexham and Flintshire and Denbighshire.

This service provides support and care to patients and their families during the last few months / weeks and days of life. The service integrates with existing agencies, i.e. Macmillan Specialist Palliative Care Team, District Nursing Teams, Marie Curie Nursing Services, and Hospices, to provide improved end of life care to patients at home.

The post holders will work directly and indirectly under the guidance of the Hospice at Home Team Leader, providing basic nursing care and holistic support to patients and their families in their own homes.

You will required to work rota'd weekends and Bank Holidays.

The successful candidate will provide high standards of care to patients at the end of life, whose preferred place of care/death is at home. Also providing emotional support for family/carers. The successful candidate would be expected to ensure privacy, respect, dignity and confidentiality is upheld at all times, and excellent communication skills are vital for this role.

This post is based within the Specialist Palliative Care Department in Wrexham Maelor Hospital and the Hospice at Home Service is offered to patients in the Wrexham, Flintshire and Denbighshire county's, and candidates must have access to a vehicle for travelling between patient's homes, in towns and rural areas.

The post requires the candidate to have an NVQ Level 2 (or equivalent) and/or working towards an NVQ Level 3 (or equivalent). Candidates should be committed to the modernisation of services and should be able to work closely within a team and alone on some occasions., If you have a strong desire to work within palliative care and possess the appropriate experience and qualifications, excellent communication and interpersonal skills, then we would love to hear from you.
