Full Job Description

The post holder works in a community house Greville Court supporting people with a Learning Disability who have complex physical health needs; mental health needs and behaviours that challenge.
All clients have impaired communication and are entirely dependant on staff to meet their personal, day to day and social care needs in order to successfully live in, and access the community.
The post holder will assist in the provision of quality assured nursing care within the residential unit and carry out tasks involving direct and personal care, support clients to access the community and to attend appointments etc, working at all times within the guidelines of the clients Care Plans and Risk Assessments, and within Health Board policies and procedures.
Care is undertaken with the support of a registered nurse and the post holder will during the period of the week be expected to work without direct supervision of a registered nurse for all or part a shift, accessing support and guidance as required on a 24 hour basis., Perform duties and activities within the framework of a care plan, maintain a high standard of care at all times, supporting the registered nurse and report any issues to the registered nurse/ team manager/ appropriate person.
Assist in ensuring that realistic individualised care plans and risk assessments are used.
Act as a key worker with an identified client ensuring person centred care is given at all times.
Support the clients with holidays and other activities within the community to ensure they have a valued lifestyle.
Drive vehicle to support clients with activities of daily living within the community.
Participate in multi-disciplinary meetings when required.
Support individuals in attending urgent, non-urgent and routine appointments.
When qualified staff not on shift staff are expected to take the role of shift leader, working within the guidelines and goals set for the clients, and health board policies and procedures.
Administers regular or 'as required' medication to clients on a daily basis, ensuring all policy and procedure is adhered to and report any adverse reactions to the registered nurse.
To complete relevant documentation electronically on a daily basis and to follow protocol and local guidelines.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable.
