Full Job Description
Are you a highly motivated individual looking for a new challenge within Health & Justice services? Do you work in a trauma informed way, with the skills to make a difference to the lives of vulnerable individuals in the community? If so, this could be the role for you. The post holder will be recruited as a Health & Justice Practitioner primarily based at either Crawley or Worthing custody suite, with the flexibility of working across Sussex, dependent on skill and service need. The successful applicants must have knowledge of working with individuals who present with a range of vulnerabilities and identification of risks with the ability to follow direction and seek support where required. The role will welcome you into a team of professionals from different backgrounds, primarily completing screenings of individual needs from within the custody environment. The post holder will be required to provide high quality information to key decision makers in youth and criminal justice agencies, including the police, courts, probation and Youth Offending Teams (YOTs), and will be responsible for completing Court Reports that may need to be taken into account when decisions about charging and sentencing are made., 1. Provide compassion to vulnerable individuals at points of acute vulnerability in the criminal justice process. 2. To support criminal justice agencies to better identify vulnerabilities in their respective environments. 3. Provide criminal justice agencies with advice and information regarding pathways to access further support. 4. To secure referrals into mainstream health and social care services and other relevant interventions and support services from a range of statutory and voluntary providers. 5. Completing the screening and management of service user need through initial information gathering, independent face to face screening demonstrating safe clinical decision making, seeking support from the H&J practitioner, identifying further assessment as indicated. 6. Screening of the individual, through the relevant health databases in consultation with other service providers.
