Full Job Description

Lead, facilitate and embed a positive, pro-active approach to supporting children with disabilities.
Maintain an up-to-date, citywide overview of all children with disabilities
Co-ordinate and facilitate effective communication between all professionals and partner agencies involved in the care planning of the child and family.
Be an advocate for children with disabilities and their families at strategic, inter-agency working groups
Co-ordinate, update and disseminate information on resources, services, support networks and contacts for children with disabilities to enable health visitors to facilitate timely access to support.
Co-ordinate and provide learning opportunities, expertise and guidance for health visiting teams in order to optimise and safeguard health outcomes for children with disabilities.
Implement, evaluate and audit agreed processes for information sharing and partnership working between health visitors and other agencies involved in managing the child with disabilities.
