Health & Wellbeing Coach - WCC PCN

We are looking for a dedicated Health and Wellbeing Coach to join our team. You will support patients, particularly those with long-term conditions, in managing their health and wellbeing. Using health coaching techniques, you will empower patients to understand and manage their conditions independently.,

• Provide personalised care and wellbeing coaching to a cohort of patients.


• Work closely with GPs and multidisciplinary teams to develop and implement patient care plans.


• Participate in multidisciplinary team meetings to discuss patient progress.


• Engage with community resources and initiatives to promote healthy lifestyles.


• Manage a caseload independently, ensuring patient outcomes are met.


• Support quality improvement projects across the PCN.
