Healthcare Assistant

The main duties of the post of Health Care Assistant involve the following: To care for patients awaiting discharge/transfer from the hospital. Ensure patients have their nutritional needs met during their stay in the discharge lounge. Ensure the patient is settled into the discharge lounge area comfortably and is aware of the facilities available. Assist 1st level nurse in order that patients receive any ongoing nursing care whilst in the discharge lounge. Liaise with Pharmacy to ensure discharge prescriptions are available prior to transport arriving and collecting medication if necessary. Ensure filing in the patients health record is carried out to meet Trust standard. Record and monitor activity data. Post holder may be re-deployed to medical wards if low patient use in discharge lounge. Post holder must have the flexibility in working hours to meet service needs. Observe the Health and Safety at Work Act and report any know defects in equipment and/or aids to the Nurse in Charge of
