Full Job Description

The role is to support Respiratory Nurses to provide excellent Respiratory management to patients in their own home or clinic setting, in order to prevent unnecessary hospital admission, and to improve patient experience as well as ensuring preferred place of care at end of life.
The post holder is expected to conduct basic venepuncture, observations including blood pressures and weight, report the outcome of assessment or tests to staff, support administration staff/ nurses in contacting patients and delivery/collection of necessary equipment and support patients undertaking pulmonary rehabilitation classes
We welcome applications from Health Care Assistants who have previous health or social care experience in Community Care, excellent communication skills, and are passionate about improving patient outcomes.
As the post is to cover the Sefton borough, the post-holder must be able to travel.
The service is currently Monday to Friday, 9am to 5pm. There are plans in the future that the service would become a 7 day service working 8am to 8pm daily., + Commit to the vision of supporting Mersey Care in becoming a leading organisation in the provision of community services, mental health care, addiction services and learning disability care, and in doing so fully utilise their skills and experience to support the

Working from clinic locations, acute hospital sites and patient's homes across the Sefton borough, the successful post-holder will work as part of the Community Respiratory Team, supporting the team's nurses to assess and treat our patients in a variety of settings.
The post-holder's duties will include but not be limited to venepuncture, patient observations and an ability to lone work in the community. Supporting the Virtual Ward programme in acute hospital sites (Southport and Aintree) and assisting the therapy team in supporting patients in Pulmonary Rehabilitation classes is part of the role. A Care certificate and venepuncture competencies would be advantageous but not essential, as this training as well as News 2 can be provided inhouse.
