Healthcare Assistant (Community)

We are looking to recruit an experienced and enthusiastic healthcare assistant, to provide holistic individualised care and support to children and young people with life-shortening conditions and their families across West Sussex and South-East Hampshire., Caseload manage, including key working, providing short fun breaks in the community and home setting, prioritising workload, escalating concerns to staff nurses

Under supervision, participate in the delivery of high quality, care to children and young people who receive short breaks and end of life care from Chestnut Tree hospice services.

Work within other areas of the Hospice as directed to do so to maintain service continuity and responsiveness e.g. in-house team.

Provide clinical care within a competency framework of delegated duties Identify and report on significant changes in children and young peoples conditions or wellbeing outside own areas of competence.

Work within own scope of competence and to be accountable for a delegated task.

Implement care plans, as delegated by Registered Nurses to meet the individual needs of each child/young person, upholding their daily routines, as far as is practical.

Chestnut Tree House is the children's hospice for East Sussex, West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-shortening conditions
