Full Job Description

As a Support Worker in the team, you will be supported by the wider multi-disciplinary team to deliver intensive, recovery focussed intervention for individuals presenting with acute clinical needs as an alternative to hospital admission. You will hold positive communication skills, enabling you to be a role model for others. You will also be able to liaise effectively with other services, both internal and external to the Trust, retaining service user focus at the heart of any clinical decision making to ensure the best outcome., Observe, report and record changes in the service user's mental health, physical health and behaviour. Identifying early signs of deterioration and change to risk presentation.

Be responsible for building a rapport with service users whilst maintaining personal and professional boundaries.

Support service users in engaging effectively with the agreed care plan formulated within multi-disciplinary meetings.. To assist the individual to access appropriate services provided on a regular and consistent basis, and to ensure the individual understands their pathway of care across sector/agency boundaries.

Positively promote service users in the community by maintaining extensive knowledge of and links with community resources. To actively support service users to use these resources, thereby countering discrimination and encouraging social inclusion.

Work closely with CMHT care co-ordinators involved in the service user's care and other staff as appropriate.

Maintain adequate records by existing policies and procedures, using appropriate paper and electronic systems. Entering appropriate details on the service users' case notes as necessary.

Participate in the review of the Acute Mental Health Team's service provided to service users by Health Care Support workers.

Undertake such other duties as may be determined from time to time within the scope of the post, as identified by the line manager.
