Full Job Description

Teamwork is an essential part of this role as you will be working as part of a multi-disciplinary team.
You will be responsible for supporting a wide range of specialty clinics and developing services according to patient demand and need.
This is an exciting opportunity for a Health Care Support Worker to gain experience in supporting an Outpatient setting.
Under the delegation and supervision of the registered healthcare practitioner, the Health Care Support Worker will carry out all of the delegated duties listed in the main responsibilities section below independently within the health care team. The post holder will work within the out-patient department as part of a team supporting patients, clinicians and colleagues during outpatient consultations and procedures. They will provide the fundamentals of care to patients in the hospital to ensure high standards are maintained in a safe environment including patient safety and to make sure that the care experience is positive by treating all patients, family, relatives and colleagues with respect, dignity and courtesy in accordance with Trust values and policies.
No duties are to be undertaken by the post holder unless they have received the correct training. All duties must be in accordance with the Band 2/3 skills matrix., 1. Will have the knowledge and skills to provide the fundamental care needs for individual patients under the direction and supervision of the registered practitioner, the following can be undertaken in the band 2/3 training period, following the GWH standards of care. These duties will be repetitive, routine and familiar, following the patient care plan and consent gained from the patient which must include;
+ Caring and communication with kindness and compassion
+ Personal Care including catheter care (if required)
+ Nutrition and Hydration support
+ Pressure Area Care
+ Collect specimens (routine bodily fluids only)
+ Moving and handling
+ Documentation including food & fluid charts and outcome sheets.
+ Environmental maintenance
+ Performing care in line with risk assessments and care plans
+ Ensure all clinical rooms are stocked up with appropriate clinical forms and patient information sheets
+ Assist with the preparation of medical notes to facilitate the running of clinics.
+ Escort to other departments (except theatre)
+ Instigate Emergency procedure
+ Completion of accident / incident forms
+ Promote person centered values and provide support for patients wellbeing and fulfilment
+ Under guidance and instruction, remove backslab cast, splints, braces and simple dressings
+ Under guidance and instruction, fit simple splints and braces
2. Achieve the desirable requirements in the training and education section of the person specification within agreed timeframe.

Current employees should be aware that this position is advertised as a Great Western Hospitals position and appointment ends any TUPE protection.
Candidates are responsible for covering the cost of their own DBS check if required: £18 for a Standard DBS check, recoverable from the first pay, or £38 for an Enhanced DBS check, divided and recoverable over the initial two pay periods. Payment for DBS checks for bank posts must be settled prior to commencement. Apprentices are exempt from DBS payment.
