Weare seeking an enthusiasticHealth Care SupportWorker,who is passionate about working in olderperson'srehabilitation.The Day Hospital provides a7-dayservice and weekend working on a rota basis is a requirement of the role.This post isBased in Christchurch DayHospital. The post also requires working in the community in patients own homes. A full driver's licence and use of a car is essential.

The Day Hospital is anaward-winningservice ofintegrated professionals. This highly motivated team specialises inolder persons care andrehabilitation,providinga holistic approachthat supports and empowers patients to achieveindependence.Previousexperience of older personin a rehabilitation environmentis essential.

A current care certificate is required for this role

Base Location: Christchurch

Key responsibilities of the position will be towork closely with themultidisciplinary teamand otherhealth careprofessionals inassistingthe assessment and treatment of the older person. Part of yourrole willbe toprovide healtheducationand exercise therapy both in theDayHospital and thepatient'shome setting.This is also an exciting opportunity to be part of a service which is growing and evolvingin the merge between Bournemouth and PooleHospitals.Applicantswill need to be motivated, highly organised, dedicated tohigh standardsof care and able to work on your own initiative as well as in a team.

· To provide a high standard of service to elderly patients through their admission and treatment within the Day Hospital and outreach service.

· To provide rehabilitation in an out-patient setting and in the patient's home environment.

· To assist the clinical leads and Multi-disciplinary team in providing efficient and cost effective service within the resources available, meeting agreed standards, objectives and statutory requirements within Christchurch Day Hospital
