Do you want to make a difference to the lives of children, young people and their families across Warwickshire? If so, then we need your help to deliver our highly regarded School Nursing Service. We are seeking a motivated and solution-focused Healthy Lifestyles Practitioner (HLP) to join our dynamic and innovative team.

Connect for Health is a school nurse-led service that supports children, young people and their families in Warwickshire. We deliver key elements of the Healthy Child programme, working to identify and meet the health and wellbeing needs of the school-aged population. We aim to give individuals the best start in life and reduce health inequalities.

The Healthy Lifestyles Practitioner role includes the development, delivery and evaluation of age-appropriate Public Health interventions to children, young people and families. In this role, under the direction, support and guidance of the Healthy Lifestyle Lead, you will deliver a range of Public Health interventions in schools and community venues across Warwickshire .You will support with weighing and measuring children as part of the annual National Child Measurement Programme (NCMP). You will also assist children and young people to complete health questionnaires.

You will have experience of working with children, young people and families and will have excellent communication skills which can be tailored to the needs of individuals. You will have experience of delivering health promotion messages to individuals and groups and will have the skills to creatively use evidence based behaviour change techniques to make activities, educational, interactive and fun. You will also be able to take into account, race, lifestyle and cultural background when developing and delivering Public Health interventions. Experience of supporting children and young people with SEND would be an advantage though not essential.

You will work both independently but also with others to support the overall delivery of Public Health interventions. You will be part of a close-knit team who is dedicated to improving the wellbeing of children, young people and families.

You will also hear the views of children and young people and will work with others to ensure that service delivery is meeting their needs.

The successful candidate will be based in one of four geographical hubs in either Nuneaton, Rugby or Stratford-upon-Avon. Public Health interventions will be delivered countywide however it is a larger number of interventions will need to be delivered in the north of the county where health outcomes are known to be worse. The working pattern is all year round.

You must possess a full driving licence, be able to travel and have access to your own transport.
