Full Job Description

We have got an exciting opportunity for a Healthy Weight Practitioner to work alongside our School Nurse Team and Weight Management Service BeeZee Bodies. This role is key in ensuring children and young people in Waltham Forest have appropriate interventions in place to support the health weight and lifestyles following on from the National Child Measurement Programme. This role requires you to work on evenings and weekends. This is an initial 6 month fixed term contract until 31 March 2025. Hours are 30 per week.,

  • Lead on the delivery of the NCMP in Waltham Forest in all mainstream and special needs schools
  • Support and guide team to ensure NCMP delivery is as planned
  • Input data onto NHSCIC database
  • Contact families who are identified as overweight or very overweight by the National Child Measurement Programme prior to them receiving their letter.
  • Make contact with 100% of Families whos child is overweight or obese, as per NCMP result.
  • Refer eligible Families with children over 5 to Childrens Weight Management Service Provider Services to complete extended brief intervention and under 5 to Best Start Service Provider.
  • Signpost to other locally useful services and support.
  • Attend training and follow guidance materials provided by the Childrens Weight Management Service Provider (BeeZee Bodies)
  • Access the National Child measurement programme database (or local database depending on local system) and details for all children identified as overweight or very overweight
  • Make calls to families identified as overweight or very overweight. If families do not answer, send text message and attempt a second call, at a different time (e.g. if called during the day, try in the evening).
  • Calls should be made at a range of times to cater for different family and work patterns including at least two evenings during the week (5pm-7pm) and a minimum of two Saturdays a month.
