Full Job Description

Imperial College Healthcare NHS Trust has a large and active Heart Failure Service which is very well regarded. The service is a fully integrated one, caring for patients both in hospital, but also in the local communities in which they live.
Our community service cares for patients within Hammersmith and Fulham and Central and West London boroughs - we have a dedicated team of heart failure nurses, pharmacists and consultants who provide care and support for patients within their own homes, in local community clinics and in ambulatory outpatient areas at St Mary's and Charing Cross hospitals.
We are looking for a new member of staff to join our busy, friendly, dedicated community team on a full time basis. The successful candidate will be responsible for the timely and accurate data capture of key data sets for submission to the national heart failure audit. The post holder will also assist the heart failure team with other administrative duties when required and can be involved in ongoing research activities if interested., + The post-holder would have specific duties to collect and input the required Heart Failure data for the National Institute for Cardiovascular Outcomes Research (NICOR).
+ Develop the data needs and subsequent submission of unit data to the databases and any other on-going research in a timely manner.
+ Attend any relevant meetings and liaise with clinical staff as required, including weekly MDT meetings with the whole team
+ To be responsible for ensuring the accuracy and completeness of data highlighting omissions and errors, and ensuring these are corrected.
+ Support the service in the development of robust data from the service's electronic database collection and submission of complete and accurate information for both the National Institute for Cardiovascular Outcomes
+ Maintains quality and content of data held within information systems
+ Regularly monitor the quality of the data on which the reports are based and validate the information from where possible from other sources
+ Supports analytical work of business area.
