Full Job Description

We have an exciting opportunity for a Heart Failure Specialist Nurse to join a supportive and inclusive team who want the best for our patients and each other.
Working within the inpatient heart failure team at Chelsea and Westminster Hospital including cross site working at West Middlesex Hospital.
The post holder should have expertise in cardiac care provision with a specific interest in Heart Failure. The post holder should have an extensive experience of cardiology and the ability to work autonomously.
As an autonomous practitioner the post holder will be required to manage their own caseloads including nurse-led clinics, telephone consultations and heart failure programmes with support from the lead Heart Failure Nurse. They will also provide support, management and education to cardiac in-patients within the Trust.
The post holder will be actively involved in the development of the heart failure services both internally and externally. This will include developing digital initiatives including Virtual wards, undertake audits, quality improvement activities and participate in the strategic development of the service.
This allies with the direction of travel within the NHS for smarter ways of working.

Main duties of the job
This is a developmental role in which the successful applicant will be supported to develop their skills and knowledge in heart failure both formally and in clinical practice. Specifically related to drug titration, patient monitoring, patient education, the assessment and management of heart failure patients and end of life care as per national and local guidelines on the care and management of heart failure.
Please see JD for further information, The inpatient Heart Failure team is a cross-site, integrated multidisciplinary team and we aim to work collaboratively to deliver care that meets best practice in heart failure across both sites of the Trust. This post will be cross-site working at Chelsea and Westminster Hospital and at West Middlesex University Hospital and the post holder will be expected to contribute to the service as a whole under the direction of the lead specialist nurse for heart failure.
This is a developmental role in which the successful applicant will be supported to develop their skills and knowledge in heart failure both formally and in clinical practice. Specifically related to drug titration, patient monitoring, patient education, the assessment and management of heart failure patients and end of life care as per national and local guidelines on the care and management of heart failure.
The post holder will be supported to manage their own caseload, which would include nurse-led clinics, virtual clinics, and telephone consultations. This would incorporate education to heart failure patients in both the inpatients and out patients setting
The post holder would also be involved in the ongoing development and facilitation of the virtual wards, the use of digital resources, the development of Remote Monitoring and utilising the ambulatory services. The post holder will provide a leadership role and forge In addition forging links with those involved in the ongoing development of the heart failure service.
The Heart Failure Specialist Nurse provides expertise in all aspects of cardiac care provision with a specific interest in Heart Failure. The post holder will be an experienced practitioner with experience of cardiology and will develop the ability to work autonomously
They will assist in undertaking regular audits, quality improvement activities and participate in the strategic development of the service. The post holder will also actively disseminate experiences through publication, conference presentations and participate in local heart failure and nursing research and national working parties.
