Heart Failure Specialist Nurse

We have a very exciting opportunity to join our integrated Heart Failure Service at Calderdale and Huddersfield NHS Trust. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for the development of a cohesive management programme for patients with heart failure in line with local and national guidelines, working across acute, community and virtual ward settings. The service will operate 7 days a week, with a view to extending hours of 8am to 8pm in the future and requires flexible working within an evolving proactive, dynamic, and committed multidisciplinary Heart Failure Team., The aim of the role is to work autonomously, within a the team, managing own caseload and to identify patients who can be supported on the Heart Failure Virtual Ward, reducing length of stay, preventing unnecessary admissions to hospital, and improving heart failure management and control with optimisation of evidence-based treatments.

We employ more than 6,500 staffwho deliver compassionate care from our two main hospitals,Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, healthcentres and in patients' homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.
