Full Job Description

An exciting opportunity has arisen for an experienced nurse to join the Heart Failure Specialist Nursing Team in South Tyneside and Sunderland NHS.

You will need to have a sound knowledge of cardiology nursing with a desire to develop skill and knowledge applicable to heart failure.

You will work autonomously and have responsibility for proactively managing a caseload of heart failure patients in a Primary and Secondary care setting., We are part of the cardiothoracic directorate, and you will work within a team of heart failure specialist nurses and the wider cardiology team based at South Tyneside and Sunderland Foundation Trust.

Provide comprehensive, effective and specialist nursing care and advice to patients, carers and health care professionals.

Assess, plan, develop and evaluate programmes of patient care in collaboration with the Heart Failure Specialist Nursing team.

Provide pharmacological management of evidence-based therapies to reduce mortality and improve life expectancy in line with national policy.
