We are offering an exciting opportunity for experienced nurse to join the Heart Failure Team on a secondment for 18 months (fixed term will be considered). The post will involve cross site working at UHCW, Hospital of Saint Cross, Coventry Community, Home visits and working from home. Applicants will need to be able to drive and have access to a car.
We are looking for a Trained Nurse with the following requirements, To support patients and carers with stable and unstable heart failure. To promote quality of life, contributing to the prevention of avoidable hospital admissions and readmissions. Review patient in hospital due to heart failure, ensuring evidence base therapy is implanted. To contribute to developing robust guidelines and protocols and providing clinical advice and education ensuring seamless pathways of care for patients with heart failure. To support advanced care planning strategies for end stage heart failure patients., + To provide and evaluate nursing care for a designated caseload of individuals with heart failure.
+ Provide clinical assessment of patients using clinical knowledge and skills as per heart failure management guidelines
+ Provide ongoing clinical support and monitoring to optimise heart failure medications in conjunction with consultant cardiologist or GP
+ Monitor clinical variations including blood biochemistry or assist the patient in monitoring clinical variations and take appropriate action to optimise treatment of the condition and prevent an avoidable admission to hospital
+ Arrange referrals for further investigations in consultation with consultant cardiologist or GP if necessary
+ To support patients in the most appropriate location i.e. ward, home, outpatient clinic, nurse led clinic and hospital at home.
+ To ensure a consistent approach to patient care, including access to and provision of appropriate patient and carer information and education programmes.

+ Experience registered nurse.
+ Experience of managing staff and clinical case load.
+ Knowledge of the management of Heart failure (HF) patients is essential.
+ The role will include reviewing and assessing HF.
