Full Job Description

Due to the retirement of 2 of our Diabetes Specialist Nurses an exciting opportunity has arisen for a motivated and dynamic Diabetes Specialist Nurse, to join the community team at Herts Valleys Integrated Diabetes Service (HIDS) in South and West Hertfordshire. The role includes clinics (face to face or telephone) and group education sessions.

The successful candidate will work as part of the wider HID Service, which is an integrated service working across community and acute settings.

Hertfordshire Community NHS Trust work very closely with our partner organisations- West Herts Teaching Hospital Trust, Hertfordshire Partnership University Foundation Trust to provide an integrated service to adults with diabetes in South and West Hertfordshire.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a payment of £2,122 per annum (pro rata where applicable).
