Full Job Description

To be based in the UHL but working collaboratively across CMG's and with community partners to identify and proactively work with those individuals who are High Frequency Users of the Emergency Department . To promote and advocate for the care provision and the smooth transition of patients in the acute care areas (Emergency Department & Emergency Floor) and ensure that patients are cared for in the appropriate environment. The practitioner will act as the clinical and professional resource to the multi- professional, multi-agency team, patients and carers identified as a High Frequency User of the Emergency Department. To be based in the UHL but working within the Primary and Secondary interface to identify and proactively work with the multidisciplinary team. To promote the care provision and the smooth transition of patients in the acute care areas (Emergency Department & Emergency Floor) and ensure the patient is cared for in the appropriate environment. The practitioner will act as the clinical and professional resource to the multi- professional team, patients and carers within their specialist service.
